While C3 does not directly provide physical healthcare, we coordinate referrals to trusted family-care providers and community partners to support each client’s whole-person wellness.

C3 Integrated Health provides person-centered treatment pathways for substance use, focusing on sustainable recovery through trauma-informed clinical intervention. Our approach honors individual experiences, fostering a sense of agency and empowerment at every stage of the journey toward long-term stability and healing.

C3 recognizes the importance of social determinants of health and provides community-based support services to promote sustained recovery and independence. These services include:

  • Peer support and recovery coaching

  • Case management and care coordination

  • Connection to community-based resources and supports

  • Supportive housing referrals and coordination

  • Workforce development and employment readiness

  • Criminal justice re-entry employment support

Integrated Wellbeing

Coordinated Whole Person Wellness
A whole-person approach combines behavioral health support with physical-health awareness and coordinated access to routine medical care. At C3 Integrated Health Clinic, we believe recovery is most effective when behavioral, emotional, physical, and social needs are considered together.
C3 provides trauma-informed behavioral health and counseling services that promote personal agency, empowerment, long-term stability, and person-centered growth. Because C3 does not directly provide physical healthcare, we partner with a trusted routine family-care provider and other qualified healthcare professionals to help clients address their physical-health needs.


Physical-health care coordination and wellness support may include referrals for:

  • Routine primary and family care

  • Health promotion and disease-prevention services-prevention services

  • Preventive health screenings

  • Nutrition and wellness education

  • Weight-management support

  • Chronic-condition monitoring and management

  • Acute illness evaluations and sick visits

  • Physical examinations

  • Vital-sign monitoring

  • Cardiovascular screening, including ECG testing

  • Hearing and vision screenings

  • Immunizations and vaccinations

  • C3 may also coordinate referrals for infectious-disease testing and treatment, including:

  • COVID-19 testing

  • HIV testing

  • Syphilis testing

  • Hepatitis C testing and treatment

These physical-health services are not performed by C3 Integrated Health Clinic. They are provided by our trusted family-care partner or another appropriately licensed healthcare provider. With the client’s authorization, C3 may assist with referrals, appointment coordination, communication between providers, and continuity of care.
